Center for Advanced Health Management honored for complex care services

VCU Health’s Center for Advanced Health Management has won a Decision Health Platinum Award. CAHM opened in 2014, providing a comprehensive care model for qualifying patients with complex care needs, including for older adults.

Decision Health is a news organization focused on financial aspects of the health care industry, and it recognized CAHM with the Platinum Award in the Publically Funded Plan — Payer Based Programs category. 

CAHM uses population health principles and an integrated continuum design to identify and better manage medical and psychosocial aspects of patients’ chronic illness and disability.

“VCU Health and Virginia Premier Health Plan together embraced the opportunity of the Commonwealth Coordinated Care demonstration for persons insured through both Medicare and Medicaid to create a new and better model of coordinated care for high-risk individuals,” said Peter Boling, M.D., chair of the Division of Geriatric Medicine, which houses CAHM. “Many of them have serious health problems as well as complex social needs and have suffered for years from fragmented, poor quality care and limited access.

“Using evidence-based practices and 30 years of prior work in this field, we invested resources to create a state-of-the art specialized hub to provide comprehensive team-based care across the continuum — office, in-home (house calls), hospital and nursing home — for individuals who are at risk. We are honored to have this work recognized.”
